  • Faculty/Staff Departure Verification Form
    Complete when an international faculty / scholar / staff member in the department is terminated from their appointment, resigns, or completes their stay.

J-1 Exchange Visitor Applications

  • DS-2019 Request Form
    To request paperwork for a new J-1 Exchange Visitor program in the Research Scholar, Professor, or Short-Term Scholar categories. This includes scholars transferring-in from another institution. To request paperwork for a J-1 Student Intern program, see the J-1 Student Intern Request Form below.

H-1B, O-1, and E-3 Applications
Please contact OIS if you have any questions about inviting individuals in these statuses.

  • H-1B Temporary Specialty Occupation Workers
    See the H-1B Guide to Hiring for details on requirements, program limits, restrictions, and processing times. To initiate the process, complete the H-1B Request Form, Deemed Export Certification Form, and H-1B Letter to the Examiner listed below and submit drafts to OIS. Included with these forms, submit a copy of the offer letter and university job description (if applicable). They may be emailed to oisscholar@uic.edu or ois@uic.edu. Once reviewed, original, hard copies will be required. 
  • O-1 Workers of Extraordinary Ability Applications
    See the O-1 Guide to Hiring for details on requirements, program limits, restrictions, and processing times. To initiate the process, please first send a copy of the individual’s most recent and complete CV to OIS for review. We will then schedule a meeting or conference call to discuss the process. Once the application has been approved for submission, send an original, hard copy of the O-1 Request Form and Deemed Export Certification Form to OIS. 
    • O-1 Request Form - For standard Initial, Extension, Amendment requests for Individuals of Extraordinary Ability. Remember to also submit a Deemed Export Certification Form.
    • Deemed Export Certification Form – Submit with the O-1 Request Form.
       
  • E-3 Australian Specialty Occupation Worker Applications
    E-3 status is very similar to H-1B status, but is reserved exclusively for Australian nationals. You may see the H-1B Guide to Hiring for basic information on the process. However, unlike H-1Bs, E-3s may only be invited for a maximum of 2 years at a time, but may be extended indefinitely, as long as the yearly cap of 10,000 visas is not met. To initiate the process, complete the E-3 Request Form below and submit a draft to OIS. Included with these form, submit a copy of the offer letter and university job description (if applicable). They may be emailed to oisscholar@uic.edu or ois@uic.edu. Once reviewed, original, hard copies will be required. 
    • E-3 Request Form – For Initial, Extension, Amendment Requests for Australian National Specialty Occupation Workers.

TN Applications
Please consult with OIS before inviting a TN worker specialty occupation worker from Canada or Mexico under the North American Free Trade Agreement (NAFTA).  

B-1/WB Business Visitor Invitations
Please consult with OIS before inviting a business visitor so that we may assess if the proposed activities fit within the B-1/WB classification. OIS will send you a draft invitation letter to assist with this assessment. See the Visitors section for more information. 

Permanent Residency (PR) Applications
Please consult with OIS before initiating any of the following forms. Requests for employment-based Permanent Residency (PR) sponsorship must be made by the department and adhere to UIC’s Permanent Residency Policy. To begin the process, you may contact OIS at ois@uic.edu, stating the desire to begin the permanent residency process for an individual in your department. Please include the individual’s most recent and complete CV and a copy of the fully executed offer letter (must include the date the beneficiary signed the letter). OIS will then schedule a meeting, or conference call, to discuss the process. Once the application has been approved for submission, send an original, hard copy request form for the agreed-upon PR category.  

  • Outstanding Researcher/Professor (EB-1)
    This is a PR category for individuals who are outstanding in their field; one may be eligible for this classification if he or she is “recognized internationally as outstanding in a specific academic area.”  To qualify, the offered position must be for either a tenured or tenure-track teaching position or a permanent research position (funding for at least 5 years or historically had been funded for at least 5 years.)  In addition, the international beneficiary must have at least 3 years of experience in teaching or research in the academic area and seeks to remain in the United States to teach or conduct research in the academic area.
  • PERM/Labor Certification Special Recruitment/“Teaching” (EB-2)
    This is a PR category for positions for college and university teachers, which takes advantage of the results of our own competitive recruitment procedures, provided a national journal advertisement was used with very particular parameters.  The teaching appointment must minimally require a Master’s degree or higher.  Submit both original forms below, after the initial consultation.
  • PERM/Labor Certification Basic Recruitment/“Non-Teaching” (EB-2 and EB-3)
    This is a PR category in which we must demonstrate to DOL that there are no U.S. citizens or permanent residents in the geographic area of employment who are able, willing, qualified & available by “testing the market.” This can include teaching positions in which no national journal ad was previously run or the 18-month window filing deadline has passed. The EB-2 subcategory is for positions which minimally require a Master’s degree. The EB-3 category is typically used for Civil Service and Academic Professional positions that require a Bachelor's degree as the minimum. Submit both original forms below, after the initial consultation.
  • National Interest Waiver Physician (EB-2)
    This is a PR category for physicians of exceptional ability holding an advanced degree. The physician must agree to work for a 5 year period of time in a designated underserved area upon approval. Submit the original forms below, after the initial consultation.
